i swallow the beams and sundrops

mom made the most awesomest pumpkin + bacon soup for lunch! it was really really really good and flavourful, can really have it every single day :) hehe anyway back to home again today. had a really good good time yesterday with friends in SoHo KL but tiring haha ♥

anyway, my second patient this week was an epileptic patient. and i think i really just got blank for awhile and didn’t take the seizure history ie the type of seizure, the cause, frequency and trigger etc etc. but also don’t know how to ask these kind of questions. anyway, one good thing from seeing real patients is that we can really go back and review each case because they are so individualised!

epilepsy denotes a group of chronic conditions whose major manifestation is the occurrence of epileptic seizures but not all seizures imply epilepsy. there are 2 classification of epilepsy seizures : partial and generalized. a serious complication of epilepsy is status epilapticus, where there are repeated seizures over a short time without a recovery period. signs of impending convulsions on the dental chair include an aura experienced by the patient, irritability and “epileptic cry”. a seizure comes with dilated pupils, muscle rigidity, loss of consciousness and ceasation of breathing.

this patient takes Tegretol which is the trade name for carbamazepine as he/she is allergic to Dilantin (Phenytoin). its dental consequences include xerostomia and microbial infection; it gives a higher tendency to bleeding and we should avoid giving erythromycin when dispensing antiobiotics as it would lead to toxic levels of carbamazepine. it is taken twice per week and functions to elevate the seizure threshold. compliance is very important in the control of seizures and am happy also because this patient really takes care of his/her health and knows how to spell out all his/her medications, and is compliant. poorly controlled epilepsy may require additional anticonvulsant.
